The Healthy Hearts Center is an outpatient center designed to improve quality of life, optimize physical function, and provide support to patients facing cardiovascular disease. There are separate services that make up the Healthy Hearts Center. These include the anticoagulation clinic (for patients on blood thinner therapy), heart failure clinic, pulmonary rehabilitation, cardiac rehabilitation, nutrition counseling and the screening center.
A multi-disciplinary team approach is used to deliver these services. The team includes referring doctors, nurse practitioners, expert cardiovascular nurses, pharmacists, nutritionists, exercise specialists and behavioral care professionals. Patients are referred to most of these services by physician order, and close communication with referring physicians is maintained.
